The Honolulu Department of Emergency Services informs that the man was brought to shore by jet ski, but his life could not be saved. Perry died as a result of a shark attack, and the cause of death was the injuries sustained.

Following the actor's death, a warning was issued about the possible danger of a repeat attack. The Daily Star reports that the man was found near Malaekahana Beach, close to Kahuku. More than one shark bite was reportedly visible on his body.

Perry was not only an actor but primarily a professional surfer. He appeared in productions such as Pirates of the Caribbean: On Stranger Tides, Charlie's Angels: Full Throttle, and Blue Crush.
